Parotidectomie exofaciale avec dissection et conservation du nerf facial - Pr Daniel Brasnu

Une tuméfaction de la région parotidienne droite est apparue chez une femme de 28 ans trois ans auparavant.
La tuméfaction a progressivement augmenté de volume, elle est indolore. La mobilité faciale est normale. Il n’y a pas d’adénopathie cervicale palpable.

Une cytoponction guidée sous échographie évoque le diagnostic d’adénome pléomorphe.
L’IRM confirme la nature parotidienne de la lésion. Elle est en hyposignal T1, hypersignal T2 avec rehaussement après injection de gadolinium.
L’indication opératoire d’une parotidectomie avec conservation du nerf facial est formelle. Une parotidectomie exofaciale avec conservation du nerf facial a été réalisée.
Le diagnostic d’adénome pléomorphe a été confirmé par l’examen histopathologique de la pièce opératoire
